Use your insurance

Visit the campus health center first before receiving any treatment off campus.

Closed weekends and school holidays.

If you are unable to visit the health center, you should use providers in the PPO network whenever possible.

To find a PPO provider, visit the PacificSource website (link below).

  1. Click on “FIND A DOCTOR OR DRUG” on the menu bar.
  2. Click on the state in which you need to receive medical care.
  3. You can search by name, specialty, procedure, or more.
  4. Enter your city or ZIP code for locations and press “Find Providers.”
  5. Select a doctor from the list, and call to make an appointment.

Your PacificSource health plan includes coverage for prescription drugs and certain other pharmaceuticals. Visit the PacificSource website (link below).

In most cases, the provider will submit the claim for you.

If you are required to pay for services up front, then you will need to complete a claim form in order to be reimbursed by the insurance company.

Visit the PacificSource website (link below) and click on “FIND A DOCTOR OR DRUG,” select “Download a Form,” select your state, and then choose “How to Submit a Claim” and follow the instructions.

Send claim form with the bills and receipts for medical treatment to:

Make sure you fill out the form completely so your claim will be processed promptly.

Keep copies of all the documents you submit for claims.

Students covered under the student insurance plan are automatically enrolled in the Scholastic Emergency Services (SES) coverage.
