Emergency Room vs. Urgent Care

In the case of an emergency, call 911 or go to the nearest emergency room (ER).

If the health center or your doctor’s office is closed and you need immediate attention but the illness or injury is NOT life-threatening, try to visit an in-network urgent care center instead of a hospital emergency room. These facilities are often open evenings and weekends, and you will usually pay less money out-of-pocket than you would at an ER. To locate a PPO provider, visit www.anthem.com/ca or call (800) 888-2108.

The following local urgent care centers are currently part of the Anthem Blue Cross network:

– Davis Urgent Care, Inc., (530) 759-9110
– West Sacramento Urgent Care, (916) 617-2377

Always check with the provider before scheduling your appointment to confirm that they are part of the Anthem Blue Cross PPO (Prudent Buyer) network before you receive treatment.

How to search for a doctor or facility

To find a PPO provider, use the Sydney Health Mobile App or visit the Anthem Blue Cross website (link below) and follow these steps:

  1. Click Find Care, located on the Menu bar toward the top right of the webpage.
  2. Under Use Member ID for Basic Search (on the right), enter AGS and click Search.
  3. You can search by City, County, or ZIP, or by doctor (name or specialty), hospital, procedure, and more.

It is best to locate an Anthem Blue Cross doctor, urgent care center, and emergency room near you before you get sick. Always verify the provider is part of the Anthem Blue Cross PPO (Prudent Buyer) Network before you receive treatment. Always check with the doctor or medical facility directly to confirm that they accept Anthem Blue Cross before you receive treatment.

Pharmacy

Information about Prescription Drugs

Pharmacy benefits are managed by Anthem Blue Cross. Please see the Anthem Blue Cross website (link below) to find a participating pharmacy near you, or call (800) 888-2108.

To fill a prescription, visit any in-network pharmacy. If you visit a non-network pharmacy, you will need to pay for the prescription in full and then submit a claim for reimbursement.

Claims

Get your medical bills paid

In most cases, the provider will submit the claim for you.

If you are required to pay for services up front, then you will need to complete a claim form in order to be reimbursed by the insurance company.

Download a claim form below, and send the completed form with all bills and receipts for medical treatment to:

Anthem Blue Cross Life and Health Insurance Company
PO Box 60007
Los Angeles, CA 90060-0007

Make sure you fill out the form completely so your claim will be processed promptly. Keep copies of all the documents you submit for claims. To check the status of a claim you submitted, call Blue Cross at (800) 888-2108.
