Eligibility Information

U-M students who graduated during the 2019-2020 academic year are eligible for the 2020-2021 plan due to the COVID crisis.

U-M students who took classes during the 2019-2020 academic year but take a leave of absence during the 2020-2021 academic year due to the COVID crisis are eligible to purchase the 2020-2021 plan.

1095-B Information

Due to changes in the health care law, the 1095-B form is no longer needed for tax purposes, so beginning in 2020, Blue Cross Blue Shield of Michigan (BCBSM) will no longer mail the form. If you would still like a copy, you can request it by choosing any of the three options below and BCBSM will provide it within 30 days.

  1. If you provided a Social Security Number when you enrolled in the insurance, log in to your online account and choose “Forms.” You will be able to download a copy of the form. If you did not provide your Social Security Number when you enrolled in the insurance, you will not be able to download a copy of the form, but you can use option #2 and #3 below.
  2. Call the phone number on the back of your enrollee ID card.
  3. Mail your request to Blue Cross Blue Shield of Michigan, 600 East Lafayette Boulevard, Detroit, Michigan 48226.

To enroll, click Enroll under the HELPFUL QUICK LINKS section below. At the time you enroll in the plan, you may also enroll your eligible dependents. Dependents may be enrolled in the plan only if the student is also enrolled in the plan.

On-campus health services
for students

All currently enrolled UM students on the Ann Arbor campus who pay the health service fee as part of tuition are eligible for services at University Health Service (UHS), and do not pay the Domestic Student Health Plan office visit copay when seen at UHS. When appropriate, UHS can bill your insurance for services not supported by the health service fee, such as medications, certain immunizations, laboratory testing, radiology and eye care.

Outside of Michigan, this plan uses the Blue Cross and Blue Shield PPO network through the BlueCard program.
