Telehealth

24 / 7 / 365 Access

Doctors are making house calls again! Included with your student insurance plan, and provided by HealthiestYou, is 24/7/365 access to board-certified physicians online or on the phone (where permitted). Totally worth it for minor stuff (like allergies, pink eye and earaches), the doctors on Telehealth can even send a prescription to your preferred pharmacy. The future is here!

This benefit is included in this plan. Separate enrollment is not required.

PPO Network

How to search for a doctor or facility

Use providers in the PPO network whenever possible.

It is best to locate an in-network doctor, urgent care center, and emergency room near you before you get sick. To find a PPO provider, call (800) 767-0700 or visit the UnitedHealthcare website www.uhcsr.com.

  1. Click on “FIND PROVIDERS” and select “Medical,” “Behavioral” or “Dental.”
  2. You can change your zip code, and search by name, specialty, facility, medical group or category.
  3. Click “Search.”
  4. Select a doctor from the list, and call to make an appointment.

Always check with the doctor or medical facility directly to confirm that they accept UnitedHeathcare before you receive treatment.

Pharmacy

Information about Prescription Drugs

To fill a prescription, you can visit any pharmacy. You will need to pay for the prescription in full, then submit a claim for reimbursement.

To submit your claims online, visit www.uhcsr.com/MyAccount.

To submit your claims by mail, send all bills pertaining to your claim to the address below. Include the patient’s name, the insured student’s name, address, SR ID number (insured’s Insurance Company ID Number) and the name of your University (Chapman University).

IMPORTANT: Claims must be submitted within 90 days after the date of service. If you do not provide this information within one year of the date of service, benefits for that service may be denied. This time limit does not apply if you are legally incapacitated.

Keep copies of all the documents you submit for claims.

When traveling away from your school or outside the U.S.

Students covered under the student insurance plan are automatically enrolled in the UnitedHealthcare (UHC) Global Program.
