This plan utilizes the Blue Cross of Idaho Preferred Blue PPO network. When you have a health concern and need to see a doctor or other provider, choose an in-network provider to save money.

Pharmacy benefits are managed by Caremark CVS. Please see the Caremark CVS website (link below) to find a participating pharmacy near you, or call (800) 447-9638.
To fill a prescription, visit any Caremark CVS network pharmacy and pay the copay. If you visit a non-network provider, you will need to pay for the prescription in full at the time of pickup, then submit a claim for reimbursement. See the link below for a prescription drug claim form.
If you are required to pay for services up front, then you will need to complete a claim form in order to be reimbursed by the insurance company.

Circle the charges on your provider’s statement that you are submitting and staple the statement to the form. The provider’s statement must indicate: the individual provider’s name or NPI number, a procedure code and diagnosis code for each service provided, the date the service was furnished, and the charge for each service. Submit a separate member claim form for each different provider.

Students covered under the student insurance plan are automatically enrolled in the Scholastic Emergency Services (SES) coverage.
